Dear Client,

As we continue to strive for excellence in providing care, we kindly request your cooperation in completing some essential documents before your next appointment at Solara Psychological Services. PLEASE NOTE: please use a laptop and desktop computer rather than a phone and tablet. 

In the following you will find a letter that explains the entire process in detail, PDF versions of our policies, and the history packet. These documents are fillable PDFs for your convenience, allowing you to complete them electronically with ease. They should be returned to us at testing@solarapsych.com prior to your next scheduled appointment. If you cannot open PDF files please download Adobe Acrobat Reader for free here: https://get.adobe.com/reader/ . This will allow you to open and complete the documents and email back. Please do not print and fill out by hand. Also the font is automatically adjust to size of box and number of characters so some font will look bigger than others and this is fine. 

*WARNING if you use google docs or apple apps it will not save your progress.

Your prompt completion and return of these documents, including the Policies and History Packet, are imperative for a seamless and efficient process during your visit. This preparation enables us to better focus on your needs and significantly reduces waiting time. If this paperwork is not completed before your initial appointment, your appointment may be rescheduled.
