Dear Parent, 

I hope this message finds you and your family well. As we continue to strive for excellence in providing care, we kindly request your cooperation in completing some essential documents before your next appointment at Solara Psychological Services. PLEASE NOTE: please use a laptop and desktop computer rather than a phone and tablet. 

Along with this email, you will find a letter that explains the entire process in detail, PDF versions of our policies, the history packet, and the Vanderbilt Behavior Rating Scales for both parents and teachers. These documents are fillable PDFs for your convenience, allowing you to complete them electronically with ease. Please download and complete all forms. 

If you cannot open PDF files please download Adobe Acrobat Reader for free here: https://get.adobe.com/reader/ .This will allow you to open and complete the documents and email back. Please do not print and fill out by hand. Also the font is automatically adjust to size of box and number of characters so some font will look bigger than others and this is fine. IF YOU ARE NOT ABLE TO COMPLETE THIS DIGITALLY, YOU ARE WELCOME TO COME INTO THE OFFICE AND COMPLETE THE DOCUMENTS ON OUR COMPUTERS, BUT THIS NEEDS TO BE BEFORE THE INITIAL INTERVIEW.

The Vanderbilt Rating Scales & BASCs are crucial for us to understand and meet your child's needs more effectively. Therefore, we ask that you complete the parent scale and forward the teacher scale to your child’s teacher. 

The BASC forms are for parent (PRS) and for teacher (TRS). Please send teachers a digital copy of the TRS so they can complete and email back to testing@solarapsych.com. Completed scales from both you and the teacher should be returned to us at testing@solarapsych.com prior to your next scheduled appointment. 

WARNING if you use google docs or apple apps it may not save your progress and this is why we ask you to complete on desktop or laptop computer.

Your prompt completion and return of these documents, including the Policies and History Packet as well as the Vanderbilt Rating Scales, are imperative for a seamless and efficient process during your visit. This preparation enables us to better focus on your needs and significantly reduces waiting time. If this paperwork is not completed before your initial appointment, your appointment will be rescheduled.
